ADMISSION REQUIREMENTS AND PROCEDURES

1. Secure application and two (2) copies of the recomendatio form from the Admissions Office.
2. Submit the following requirements:
   - Accomplished application form with 2X2 ID picture.
   - Two MOFM recommendation forms (from the Principal, Counselor or Teacher).
   - Original report card. Form 138 (for high school graduates).
   - Complete records of collegiate grades certified by the registrar (for transferees).
   - Study permit ACR (for foreign students).
3. Interview conducted by MOFM Admissions office and enrollment permit is given.
